1. Guidance on Procedure of the Request for Disclosure, Correction, etc. of Retained Personal Data

We will respond to requests made by individual in accordance with the Personal Information Protection Act. Applicable requests for retained personal data of individual are: “notification of the purpose of use”, “disclosure”, and “disclosure of third-party provision record” (hereinafter referred to as “Request for Disclosure”); and “correction, addition, deletion”, “utilization cease or deletion“, and “third-party provision cease” (hereinafter referred to as “Request for Correction”).

Persons who can make a request

The individual, his/her legal representative, or his/her mandatary can make a request.

Fee

［ Request for Disclosure ］
As a service charge, 800 yen (tax-included) is required to make each request. We will contact you the payment details after receiving and confirming the necessary documents listed below.
［ Request for Correction ］
No charge.

Reception method

Please enter necessary matters in the Request Form for Retained Personal Data (you may download from "Necessary documents when making a request" below) and mail it with the necessary documents to the following address. Please mail the documents through a mailing service that ensures delivery tracking and confirmation, such as registered mail. Kindly indicate in red on the envelope: “Enclosed: Request for retained personal data”.

Necessary documents when making a request

Please send the following documents to the address written above.

1	Always required	Request Form for Retained Personal Data *Please attach the documents listed in 2, 3, and 4 (excluding the Letter of Attorney) to the Request Form.

4		Request by mandatory	Documents necessary for identity verification of the mandatory
			Letter of Attorney from the individual (only document stamped with a seal that has been officially registered is accepted)
			Seal registration certificate (This pertains to the registered seal of the individual as affixed to the Letter of Attorney. Within 3 months from issuance.)

Necessary documents for identity verification of individual, representative, and mandatory

Please submit a copy of one of the following documents for identity verification of the individual, representative, and mandatory.
Please ensure that the submitted document includes the same address as your current residence (document delivery address).

Driver’s License	Only those within the expiration date. Please ensure that the copy clearly shows your current address.
Health Insurance Card	Only those within the expiration date. Please ensure that the copy clearly shows your current address.
Front side of Individual Number Card (My Number Card)	Please submit a copy of the front side which includes your address. We do not accept a copy of the reverse side that contains your individual number (My Number).
Certificate of Residence	Only those within 3 months from issuance. Please send the one with only required items such as name, address, etc.

Response to the request

• We will respond to you by mail (which leaves the delivery record)
• The response will be sent to as follow.
  • (1) Request by individual: To the individual
  • (2) Request by individual’s legal representative: To the individual’s legal representative
  • (3) Request by individual’s mandatory: To the individual’s mandatory

• For requests concerning the "disclosure" and "disclosure of third-party provision" of retained personal data, the method of disclosure will be either by written documents or by providing an electronic records (saved on a CD-ROM). The document or CD-ROM will be sent to the address provided. If the method is not selected at the time of request, we will provide you in a written documents.
• We will response without delay from receiving the request. Please note that the response will be delayed during the New Year holidays, Golden Week, summer vacation, etc.

Handling of personal information collected for the request

• Personal information we acquired in the request will be used only to the extent necessary to respond to the request.
• The identification documents and the seal registration certificate you submitted will be returned to you along with our written response to your request by mail.

Other notes

If there are any errors in the Request Form, or if the required documents (such as identification or a letter of attorney) are incomplete, the submitted documents will be once returned to you. Additionally, if there are any unclear points in the content, we may contact you for clarification.
Please note that we may not be able to respond to your request under certain cases specified below. In such cases, we will notify you accordingly, including the reason. In addition, please be advised that the prescribed fee is non-refundable, even if we are unable to fulfill your request.

• Documents and the contents regarding request
  • (1) When the identity of the individual cannot be verified by the submitted identification documents.
  • (2) When the authority of the representative cannot be confirmed at the time of application by the representative.
  • (3) When there are deficiencies in the prescribed application documents.
  • (4) When the prescribed fee has not been paid (in case of Request for Disclosure).
  • (5) When the subject of the request does not fall under “retained personal data”.
• Request for “disclosure” or “disclosure of third-party provision record” of retained personal data
  • (1) When it threatens to infringe on any rights and interests of the individual or a third party including human life, health, properties, etc.
  • (2) When it threatens to significantly hinder proper performance of our business operations.
  • (3) When it would result in a violation of laws and regulations other than Personal Information Protection Act.
• Request for “notification of the purpose of use” or “Request for Correction” of retained personal data

  Based on the Personal Information Protection Act, we may not notify the purpose of use or make correction, etc.
