Request for Disclosure, Correction, etc. of Retained Personal Data

You may confirm the request for disclosure, correction, etc. of retained personal data obtained by Yamaha Motor Co., Ltd. (collectively, the “Company”, “we”, “us”, and “our”).

The individual or representative whose retained personal data is acquired by us can make a request for disclosure, correction, etc. of retained personal data through the following procedure. In all cases, we will appropriately respond to it in accordance with the purport of the Personal Information Protection Act.


1. Guidance on Procedure of the Request for Disclosure, Correction, etc. of Retained Personal Data

We will respond to requests made by individual in accordance with the PIPA. Applicable requests for retained personal data of individual are: “notification of the purpose of use”, “disclosure”, and “disclosure of third party provision record” (hereinafter referred to as “Request for Disclosure, etc.”); and “correction, addition, deletion”, “utilization cease or deletion “, and “third party provision cease” (hereinafter referred to as “Request for Correction, etc.”)

Persons who can make a request
The individual, his/her legal representative and his/her mandatary of the individual can make a request.
Fee
[ Request for Disclosure, etc. ]
As a service charge, postal money order (within 6 months from issuance) * of 800 yen (tax-included) is required to make each request. Please send those with the necessary documents listed below when requesting for disclosure. We may charge extra fee if necessary.
*Postal money orders can be purchased at post office or Japan Post Bank.

No charge.
Reception method
Please enter necessary matters in the Request Form for Retained Personal Data (You may download from "Necessary documents when making a request" below.) and mail it with necessary documents, etc., to the following desk in charge. In addition, please mail the documents using priority mail, etc. that leaves the delivery record. It will be appreciated if you could write “Request for retained personal data enclosed” on the envelope in red color.

We inform division in charge and its contact information when we collect personal information from customers or send direct mail, etc.

  • If the individual is underage: Family register or certificate of residence which describe the individual and the representative, and the relationship.
  • If the individual is adult ward: Certificate of registered matters
  • If the individual is minor ward: Family register

Request by mandatory
Documents necessary for identity verification of the mandatory (*)
Letter of Attorney from the individual (only the ones with seal that has been seal registered).
Seal registration certificate (The one related to individual’s seal which has been stamped on the Letter of Attorney. Within 3 months from issuance.)

Please send a copy of one of the following documents when requesting to verify individual, representative, and mandatory.
In addition, please send the documents with the same address as your current address (document delivery address).
Driver’s license Only those within the expiration date.
Please send a copy that we can recognize the address.
Health insurance card Only those within the expiration date.
Please send a copy that we can recognize the address.
Front side of Individual Number Card (My Number Card) Please send a copy of the front side which has the address. We do not accept a copy of the back side with My Number Card.
Certificate of residence Only those within 3 months from issuance. Please send the one with only required items such as name, address, etc.
Response to the request
  • We will response by mail (which leaves the delivery record)
  • The response will be sent to as follow.
    • (1) Request by individual: To the individual
    • (2) Request by individual’s legal representative: To the individual’s legal representative
    • (3) Request by individual’s mandatory: To the individual
  • Each request of "disclosure" and "disclosure of third party provision" of retained personal data will be responded by delivering written documents or the provision of electromagnetic records (CD-ROM which contains the electromagnetic records). We will use either way and mail a document or CD-ROM to the address provided. If the method is not selected in the request, we will response by mailing the documents.
  • We will response without delay from receiving the request. Please note that the response will be delayed during the New Year holidays, Golden Week, summer vacation, etc.
Handling of personal information collected for the request
  • Personal information we acquired in the request will be used only to the extent necessary to respond to the request. After the response, personal information will be stored for a specific of period and discarded.
  • Please note that submitted documents will not be returned.
Other notes
We may not respond to the request in the following cases. If we cannot respond to the request, we will notify of that effect with the reason. In addition, please note that even if we cannot respond to the Request for Disclosure, etc. we do not return the service charge.
  • Documents and the contents regarding request
    • (1) When we cannot identify the individual by the submitted identity verification;
    • (2) When the application is made by a representative, we cannot confirm the authority of representation;
    • (3) When there is any fault in the prescribed application document;
    • (4) When the prescribed fee is not paid (only Request for Disclosure, etc.);
    • (5) The target of the request does not fall under “retained personal data”;
  • Request for “disclosure” or “disclosure of third party provision record” of retained personal data
    • (1) When it threatens to infringe on any right and interest of the individual or any third party including human life, health, properties and others;
    • (2) When it threatens to significantly hinder proper performance of our services;
    • (3) When it becomes any violation of laws and regulations other than Personal Information Protection Act.
  • Request for “notification of the purpose of use” or “Request for Correction, etc.” of retained personal data

    Based on the Personal Information Protection Act, we may not notify the purpose of use, make correct, etc.
